How to register as a patient:
1. If you live in an OL8 postcode please call us on 0161 622 2760 or come into the surgery* to book an appointment for a new patient health check (about 20 mins) with one of our Healthcare Assistants, followed by an introductory appointment with a regular Doctor.
2. Pick up your new patient registration pack from the surgery or download, print off and fill in the online form below. You will need the new patient registration form and to read the information about Summary Care Records, a nationwide scheme to enable basic medical information to be accessed in an emergency. You can opt out of the Summary Care Record scheme by downloading the form attached.
3. Bring your completed forms and id with you to your new patient appointment. Over 16s will be asked to bring a urine sample (bottles available from reception) and over 40s will be asked to have a fasting blood test. You can be seen individually or as a couple or family if you prefer.
4. Your registration is complete. We will arrange for notes from your previous doctors to be forwarded to us automatically after your first appointment with us; you don’t need to contact them to let them know you’ve moved. Bring details of any previously prescribed medication with you to your appointment with the doctor and we will transfer it onto our system so you don’t miss out on repeat prescriptions.
